13U Phase 1 Practice Schedule Form

Please fill out the form below listing your first and second preference for age group practices for your daughter for the Phase 1 portion of the season between Sept. 14 and Oct. 31. (Phase 1 practices begin Sept. 8; our practices during the week of Sept. 8 will take place outside on the sand volleyball courts at Four Lakes Resort, 5750 Lakeside Dr. in Lisle.)

Spaces for athletes in each age group practice will be filled first come, first served. Please note: 13-1 and 13-2 are merely generic names for each practice group; 13-1 does not represent a higher-level group than 13-2.
